Job Summary

LifeBridge Health is seeking a skilled Addictions Counselor to join our team at Grace Medical Center. As a key member of our Assertive Community Treatment program, you will provide case management, rehabilitative, individual and group services to dually diagnosed clients.

The ideal candidate will have a strong understanding of substance abuse and mental health principles, as well as excellent communication and interpersonal skills. You will work closely with consumers to establish rapport, implement clinical interventions, and conduct substance abuse assessments in the community.

Responsibilities will include:

  • Providing individual and group counseling services to clients
  • Conducting substance abuse assessments and developing treatment plans
  • Establishing and maintaining relationships with community partners and resources
  • Participating in crisis management and intervention as needed
  • Preparing reports and maintaining accurate records

Requirements

To be successful in this role, you will need:

  • A Bachelor's degree in psychology or a related field

  • CSC-AD or CAC-AD Certification preferred
