Salary RangeWe offer a competitive salary range for this role, estimated at $65,000 - $80,000 per year, based on the national average for a Geriatric Care Coordinator in the United States.
About the RoleThis is an exciting opportunity to join our team as a Geriatric Care Coordinator, working closely with our residents, families, and healthcare professionals to deliver high-quality care and support. Your primary responsibilities will include:
- Evaluating resident needs and developing service plans in accordance with state regulatory requirements
- Demonstrating a strong understanding of geriatric care principles and best practices
- Directing and supervising daily work assignments of care associates
- Evaluating resident medical concerns and collaborating with ED, family, and MD as necessary
You will also have opportunities to:
- Encourage teamwork through cooperative interactions with all departments
- Monitor infection control and sharps disposal programs
- Participate in direct resident care as needed
- Provide support to care associates regarding assignment questions or concerns
To be successful in this role, you will need:
- A minimum of two years' experience in a geriatric setting, with previous health experience in hospital and/or community settings
- A current RN license and graduation from an accredited Registered Nursing Program
- The ability to communicate effectively in English both orally and in writing
