Job Overview:

Telligen is seeking a skilled Assessment Specialist to join our team. As an Assessor Reviewer, you will conduct thorough assessments for Medicaid beneficiaries with intellectual and developmental disabilities.

A key aspect of this role is conducting in-person assessments at the individual's home or community settings, unless health or safety concerns arise, in which case virtual assessments will be conducted.

The assessment results must be submitted within 1-2 days of the interview, and the assessment report should be shared with the client within 10 business days.

This position involves working in the field daily, completing assessments, documenting results, and providing ongoing education to clients.

Essential Functions:

  • Perform in-person and virtual assessments for individuals with disabilities.
  • Collaborate with healthcare providers to create treatment plans as required by certain contracts.
  • Document assessment results and negotiate service plans as needed.
  • Provide educational support and communicate with healthcare providers to ensure appropriate care.

Requirements:

  • Hold a four-year degree in social services or a related field.
  • Be able to pass and maintain a SIS certification, which will be provided by Telligen.
  • Have at least one year of experience working with individuals with intellectual and developmental disabilities.
  • Possess reliable transportation, a stable internet connection, and proficiency in written and spoken English.
  • Experience in human services with the ID/DD Medicaid population is highly preferred.
  • Maintain a valid driver's license and pass a motor vehicle background check.
  • Reside in one of the specified areas: Eastern Shore, Central Region, or Southern Region.
