Job Overview

About the Position

As a Perinatal Outreach Nurse, you will be responsible for:

  • Conducting home visits to pregnant and parenting mothers using the Nurse Family Partnership model
  • Developing and implementing client plans in collaboration with the Home Visit Coordinator
  • Documenting client interactions, including progress notes and assessment reports
  • Participating in weekly team meetings and contributing to quality improvement initiatives
  • Maintaining accurate records of client data and program outcomes
Benefits and Compensation

We offer a competitive salary of $65,000 - $75,000 per year, depending on experience. Additionally, you will receive a comprehensive benefits package, including medical, dental, and vision insurance, as well as paid time off and holidays.
