Case Manager

Felton Institute is seeking a skilled Case Manager to join our team. As a Case Manager, you will provide low-level case management support to a broad range of clients and situations without police or EMS personnel. You will work under the direct supervision of the AQCRT Program Director and be responsible for providing on-site crisis assessment, de-escalation, and conflict mediation. You will also support linkages that can address clients' goals, including engaging in mental health, substance use treatment, and employment.
Key Responsibilities
• Perform intake and comprehensive assessments with adults and Transitional Age Youth (TAY) who may have substance use disorders.
• Provide case management services, including evaluations, service plan development, and assisting clients with accessing housing, financial, medical, and other needed services.
• Provide individual therapy to stabilize clients and prevent decompensation.
• Conduct community outreach and engagement to safely provide services to clients within the community.
• Maintain client records in accordance with Felton Institute's mandated standards and ACPD requirements.
• Contribute to team meetings and case conferences through presentation of cases, discussion of clinical issues, and exchange of information and ideas.
Requirements
• A minimum of 2 years of case management experience with an adult and TAY population, behavioral health, and substance abuse, and justice-impacted populations preferred.
• A minimum of 1 year of experience working with a justice-involved population preferred.
• Bachelor's degree in social work, counseling, or psychology preferred.
• Bi-lingual/Bi-cultural Spanish-speaking case managers encouraged to apply.
• Ability to work cooperatively on a multidisciplinary team and with clients of diverse ethnic and cultural backgrounds.
• Familiarity with evidence-informed and evidence-based interventions to address client needs and goals.
• Ability to meet with community partners and other social service agencies.
• Meet documentation and other deadlines, comply with requirements for Medi-Cal, when applicable.
• Perform administrative tasks within timelines, including chart completion and meeting attendance.
• Connect clients with short and long-term support services, including housing assistance, public transportation vouchers, emergency food gift cards, and personal grooming and hygiene products.
• Assist clients in applying for public benefits.
• Ability to develop Individualized Intervention Plans that include service linkage, as well as intervention and personal change goals.
• Working knowledge of computer software applications, including Microsoft Word, Excel, Access, PowerPoint, and Adobe Acrobat.
• Strong organizational and time-management skills.
• Strong communication and interpersonal skills.
• Ability to prioritize projects and follow through.
• Ability to maintain proper documentation as determined by programmatic requirements.
• Ability to be a team player with a welcoming attitude.
