Job Summary

We are seeking a highly motivated and compassionate Perinatal and Pediatric Community Health Worker to join our team at LifeLong Medical Care. As a key member of our healthcare team, you will provide essential support to perinatal clients and their infants, promoting healthy outcomes and empowering them to take control of their health.

Responsibilities
  • Manage a caseload of perinatal clients and their infants, providing case management services, outreach, and health education.
  • Coordinate cases with social service agencies and medical providers to ensure comprehensive care.
  • Refer clients to appropriate support services and provide thorough documentation of CPSP visits and patient interactions.
  • Support and co-facilitate health education classes, promoting healthy behaviors and outcomes.
  • Attend team and staff meetings, and participate in community outreach and marketing activities to promote our services.
Qualifications
  • Demonstrated communication skills, both oral and written.
  • Able to prioritize competing work demands and tasks from clients and staff.
  • Able to work effectively and calmly under pressure in a positive, friendly manner.
  • Demonstrated ability and sensitivity working with diverse populations, including those with disabilities, homelessness, substance use, HIV/AIDS, and psychological impairments.
Requirements
  • Bachelor's Degree and at least one year of experience in perinatal or maternal and child health, or a high school diploma with at least two years of experience and completion of a perinatal CHW training program.
  • Experience working in a perinatal program as a Perinatal Community Health Worker or working for a non-profit community clinic and/or other non-profit social service organization.
  • Bilingual English/Spanish preferred.
