We are a healthcare organization seeking to expand our team with an additional Nurse Practitioner.
Our Family Medicine Clinic is dedicated to delivering exceptional and personalized healthcare services to our patients, emphasizing value-based care.

As a Nurse Practitioner, you will be an integral part of our core provider team, undertaking a variety of responsibilities that will help us maintain our high standards of patient care.
Key Responsibilities:
- Deliver comprehensive office-based primary care, including preventative screenings, assessing patient complaints, gathering medical histories, conducting physical examinations, documenting care plans, maintaining problem lists, and vaccination records, ordering diagnostic tests, and providing patient education.
- Improve the coordination of care by integrating records from acute, specialist, and post-acute visits into the clinical documentation and deriving clinical insights.
- Ensure high-quality care delivery and accurate documentation to uphold excellent quality scores within commercial and public payer ACO programs.
- Optimize patient care efficiency to reduce wait times and meet practice productivity targets.
- Enhance patient satisfaction through evidence-based education, collaborative decision-making, and exceptional bedside manner.
Required Qualifications:
- Completion of an Accredited Nurse Practitioner Program and current licensure to practice in Texas OR graduation from an accredited Physician Assistant Program, Board certification from NCCPA, and licensure from the Texas Medical Board.
- Preferred experience in a primary care or similar healthcare setting.
- Familiarity with documenting clinical information in an electronic health record (EHR) system.
- Strong written and verbal communication skills.
- A collaborative mindset and a positive approach.
- Alignment with ACO principles of delivering high-quality, high-value care.
