Medical Social Worker - Patient Care Specialist

Reports to:

Clinical Supervisor

Position Summary:

The Medical Social Worker - Patient Care Specialist is responsible for the implementation of standards of care for medical social work services at Excelin Health. This role requires a strong understanding of social work principles and practices, as well as excellent communication and interpersonal skills.

Key Responsibilities:
  • Patient Care:
    • Conduct thorough assessments of patients' psychosocial status related to their illness and environment, and communicate findings to the registered nurse.
    • Develop and implement social work plans based on evaluation findings, ensuring seamless integration with the patient's overall care plan.
    • Maintain accurate and up-to-date clinical records on all patients referred to social work services.
    • Provide information and referral services to patients and families/caregivers regarding practical and environmental needs, promoting optimal health outcomes.
    • Collaborate with healthcare teams to provide information and support to patients and families/caregivers, fostering a culture of patient-centered care.
    • Serve as a liaison between patients and families/caregivers, and community agencies, ensuring timely and effective communication.
    • Maintain collaborative relationships with Excelin Health personnel to support patient care and advance organizational goals.
    • Develop and maintain contracts with public and private agencies as resources for patient and organizational support.
    • Participate in in-service education programs to enhance knowledge and skills, staying current with best practices in medical social work.
    • Contribute to the development of the total plan of care and case conferences as required, ensuring comprehensive and coordinated care.
    • Assist physicians and other team members in understanding significant social and emotional factors related to health problems, promoting informed decision-making.
    • Participate in discharge planning, ensuring a smooth transition of care and optimal patient outcomes.
    • Perform other duties as delegated by the Clinical Director/Supervisor, supporting the achievement of organizational objectives.
    Position Qualifications:
    • Education:
      • Graduate of a bachelor's program in social work accredited by the Council on Social Work Education.
    • Licensure:
      • Licensed as a licensed clinical social worker by the Board of Behavioral Sciences.
    • Experience:
      • Minimum of one year's experience in healthcare, with a focus on medical social work.
      • Experience in a home health care setting is preferred.
    • Certifications:
      • Possesses and maintains current CPR certification.
    • Other Requirements:
      • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
    Skills and Abilities:
    • Communication:
      • Demonstrates excellent verbal and written communication skills, with the ability to effectively interact with patients, families/caregivers, and healthcare teams.
    • Organization:
      • Displays strong organizational skills, with the ability to prioritize tasks and manage multiple responsibilities.
