Job Summary

We are seeking a skilled House Calls Nurse Practitioner to provide comprehensive medical care to seniors and patients with complex needs in their homes or residential facilities.

As a House Calls Nurse Practitioner at Abode Care Partners, you will perform evaluation and treatment of chronic and urgent illnesses, collaborate with other healthcare professionals, and ensure comprehensive patient records are maintained.

About You
  • Active state Nurse Practitioners license
  • Current Nurse Practitioner National Board Certification
  • Active state DEA licensure
  • Master's degree in Nursing (MSN)
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred

To be successful in this role, you should possess excellent communication and interpersonal skills, as well as strong time management skills. Proficiency in medical instruments and equipment, computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications are also essential.
