House Calls Nurse Practitioner Role

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Abode Care Partners. As a key member of our medical services team, you will provide top-notch care to seniors and patients with complex needs in their homes or assisted living facilities.


Responsibilities:
  • Provide evidence-based medical care, including treating chronic and urgent illnesses, collaborating with healthcare professionals, and maintaining comprehensive patient care plans.
  • Deliver high-quality patient care, ensuring the best possible outcomes for those under your care.
  • Collaborate with caregivers and other healthcare professionals to provide interdisciplinary care and maintain a comprehensive plan of care.

Qualifications:
  • Active state Nurse Practitioners license
  • Current Nurse Practitioner National Board Certification
  • Active state DEA licensure
  • Master's degree in Nursing (MSN)
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
  • Strong interpersonal communication skills – verbal and written
  • Excellent time management skills with accountability for practitioner's daily workflow

What We Offer:
  • A competitive salary range of $100,000 - $120,000 per year, based on performance and experience
  • A flexible schedule and mileage reimbursement for certain roles and delegated activities
