Residential Rehabilitation Specialist

Prologue Inc. is seeking a skilled Residential Rehabilitation Specialist to join our team in Baltimore County. As a key member of our Residential Rehabilitation Program, you will play a vital role in creating and implementing individual rehabilitation plans for individuals recovering from mental illness.
Key Responsibilities:
- Develop and implement individual rehabilitation plans to support clients in achieving their goals
- Facilitate group and individual activities to promote skill development and independence
- Provide transportation to assist clients in accessing community resources
- Document client progress and complete incident reports as needed
- Collaborate with supervisors and colleagues to ensure seamless service delivery
Requirements:
- High school diploma required; experience in human services delivery preferred
- Strong verbal communication skills and ability to respect client choice and autonomy
- Ability to work effectively as part of a team and maintain a clean driving record
