Job Summary

Economic Opportunity Council of Suffolk Inc is seeking a skilled Healthcare Navigator to join our SSVF Program team. As a Healthcare Navigator, you will play a critical role in connecting Veterans with VA healthcare benefits and community health care services.

Key Responsibilities
  • Non-Clinical Assessment: Conduct thorough assessments to understand Veterans' situations, potential barriers to care, and the impact of such restrictions on their ability to access and maintain health care services.
  • Client-Centered Care: Work closely with Veterans to assist them in communicating their preferences in care and personal health-related goals, facilitating shared decision-making of their care.
  • Care Coordination: Provide comprehensive case management and care coordination across episodes of care, acting as a health coach to support Veterans in optimizing treatment interventions and outcomes.
  • Interdisciplinary Collaboration: Collaborate with other disciplines involved in providing care, regularly consulting with other team members and assessing and addressing the needs of Veterans.
  • Documentation and Confidentiality: Maintain accurate and timely clinical documentation and ensure consumer confidentiality.
  • HMIS Tracking: Document all contacts with Veterans in the HMIS tracking system in a timely and accurate manner.
  • Fiscal and Billing Procedures: Ensure accuracy and consistency with the agency's fiscal and billing procedures.
  • Transportation and Safety: Transport Veterans in a safe, cautious, and responsible manner, adhering to fire and life safety standards.
  • Staff Meetings and Training: Participate in all appropriate staff, supervision, and training meetings, and assume other duties as assigned by the SSVF Program Manager.
Requirements
  • Excellent Communication Skills: Possess excellent verbal and written communication skills.
  • Organizational Skills: Demonstrate strong organizational skills and meticulous attention to detail.
  • Team Player: Exhibit a team player attitude with excellent listening skills.
  • Technical Skills: Be proficient in using Word, Excel, and PowerPoint.
  • Transportation: Possess a car for visits to Veterans in their own homes or at other locations.
  • Driver's License and Insurance: Hold a valid New York State Driver's License and own insured Vehicle with 100/300 bodily injury liability insurance.
  • PPD Test: Require a PPD (TB) test with proof of results.
Preferred Experience
  • Healthcare Knowledge: Possess knowledge of healthcare systems, including VA healthcare, Medicare, Medicaid, and private insurance, as well as eligibility criteria, enrollment processes, and benefits coverage.
  • Veterans' Healthcare Needs: Be familiar with veterans' unique healthcare needs, including those related to service-connected disabilities, combat exposure, PTSD, traumatic brain injury, and other military trauma.
  • Communication and Interpersonal Skills: Demonstrate strong communication and interpersonal skills, with the ability to establish rapport and build trust with clients from diverse backgrounds.
  • Interdisciplinary Collaboration: Have experience working collaboratively with interdisciplinary teams and community partners to enhance veterans' access to care and support.
Education and Training

Masters level social work degree from an accredited college or university in social work or a related field, or a Bachelor's level social work degree from an accredited college or university in social work or a related field with 3-4 years experience working with at-risk populations.
