Job Title: Healthcare Navigator

We are seeking a skilled and compassionate Healthcare Navigator to join our team at the Economic Opportunity Council of Suffolk Inc. This role is a critical part of our mission to provide supportive services to veteran families.

Job Summary:

The Healthcare Navigator will provide non-clinical assessments, connect veterans to VA health care benefits or community health care services, and work closely with veterans to facilitate shared decision-making of their care. This role requires strong organizational skills, excellent communication and interpersonal skills, and the ability to establish rapport and build trust with clients from diverse backgrounds.

Responsibilities:
  • Provide non-clinical assessments to understand the veteran's situation, potential barriers to care, and the causes and impact of such restrictions on their ability to access and maintain health care services.
  • Work closely with veterans to assist them in communicating their preferences in care and personal health-related goals to facilitate shared decision-making of their care.
  • Provide comprehensive case management and care coordination across episodes of care and act as a health coach by proactively supporting the veteran in optimizing treatment interventions and outcomes.
  • Collaborate with other disciplines involved in providing care, regularly consult with other team members, and appropriately assess and address the needs of the veteran.
  • Document service delivery, maintain accurate and timely clinical documentation, and ensure consumer confidentiality.
  • Document all contacts with veterans in the HMIS tracking system in a timely and accurate manner.
  • Ensure accuracy and consistency with the agency's fiscal and billing procedures.
  • Transport veterans in a safe, cautious, and responsible manner.
  • Implement fire and life safety standards.
  • Participate in all appropriate staff, supervision, and training meetings.
  • Assume other duties as assigned by the SSVF Program Manager to complete tasks for the program.
Requirements:
  • Excellent verbal and written communication skills.
  • Strong organizational skills and meticulous attention to detail.
  • Team player with excellent listening skills.
  • Proficient in using Word, Excel, and PowerPoint.
  • A car for visits to veterans in their own homes or at other locations.
  • Valid New York State Driver's License and own insured vehicle with 100/300 bodily injury liability insurance.
  • PPD (TB) test required with proof of results.
Preferred Experience:
  • Knowledge of healthcare systems, including VA healthcare, Medicare, Medicaid, and private insurance, as well as eligibility criteria, enrollment processes, and benefits coverage.
  • Familiarity with VA benefits, programs, and services available to veterans.
  • Familiarity with veterans' unique healthcare needs, including those related to service-connected disabilities, combat exposure, PTSD, traumatic brain injury, and other military trauma.
  • Strong communication and interpersonal skills, with the ability to establish rapport and build trust with clients from diverse backgrounds.
  • Experience working collaboratively with interdisciplinary teams and community partners to enhance veterans' access to care and support.
Education/Training Requirements:

Master's level social work degree from an accredited college or university in social work or a related field, or a Bachelor's level social work degree from an accredited college or university in social work or a related field and 3-4 years of experience working with at-risk populations.
