Healthcare Navigator

We are seeking a skilled and compassionate Healthcare Navigator to join our team at the Economic Opportunity Council of Suffolk Inc. This role is a critical part of our mission to provide supportive services to veteran families.
Job Summary:The Healthcare Navigator will provide non-clinical assessments, connect veterans to VA health care benefits or community health care services, and work closely with veterans to facilitate shared decision-making of their care. This role requires strong organizational skills, excellent communication and interpersonal skills, and the ability to establish rapport and build trust with clients from diverse backgrounds.
Responsibilities:- Provide non-clinical assessments to understand the veteran's situation, potential barriers to care, and the causes and impact of such restrictions on their ability to access and maintain health care services.
- Work closely with veterans to assist them in communicating their preferences in care and personal health-related goals to facilitate shared decision-making of their care.
- Provide comprehensive case management and care coordination across episodes of care and act as a health coach by proactively supporting the veteran in optimizing treatment interventions and outcomes.
- Collaborate with other disciplines involved in providing care, regularly consult with other team members, and appropriately assess and address the needs of the veteran.
- Document service delivery, maintain accurate and timely clinical documentation, and ensure consumer confidentiality.
- Document all contacts with veterans in the HMIS tracking system in a timely and accurate manner.
- Ensure accuracy and consistency with the agency's fiscal and billing procedures.
- Transport veterans in a safe, cautious, and responsible manner.
- Implement fire and life safety standards.
- Participate in all appropriate staff, supervision, and training meetings.
- Assume other duties as assigned by the SSVF Program Manager to complete tasks for the program.
- Excellent verbal and written communication skills.
- Strong organizational skills and meticulous attention to detail.
- Team player with excellent listening skills.
- Proficient in using Word, Excel, and PowerPoint.
- A car for visits to veterans in their own homes or at other locations.
- Valid New York State Driver's License and own insured vehicle with 100/300 bodily injury liability insurance.
- PPD (TB) test required with proof of results.
- Knowledge of healthcare systems, including VA healthcare, Medicare, Medicaid, and private insurance, as well as eligibility criteria, enrollment processes, and benefits coverage.
- Familiarity with VA benefits, programs, and services available to veterans.
- Familiarity with veterans' unique healthcare needs, including those related to service-connected disabilities, combat exposure, PTSD, traumatic brain injury, and other military trauma.
- Strong communication and interpersonal skills, with the ability to establish rapport and build trust with clients from diverse backgrounds.
- Experience working collaboratively with interdisciplinary teams and community partners to enhance veterans' access to care and support.
Master's level social work degree from an accredited college or university in social work or a related field, or a Bachelor's level social work degree from an accredited college or university in social work or a related field and 3-4 years of experience working with at-risk populations.
