Responsibilities
  • Conduct in-home patient visits to assess and address their physical therapy needs.
  • Record and report patient progress to licensed professionals, ensuring continuity of care.
  • Coordinate patient care with other healthcare providers, offices, and physicians to enhance overall care.
  • Utilize therapy equipment and instruct patients and their families on prescribed therapeutic activities.
Requirements
  • Must be a licensed Physical Therapist Assistant (PTA) in the state of employment.
  • At least one year of experience as a licensed PTA is required.
  • Medicare home health or hospice experience is preferred.
