Job Summary

We are seeking an Occupational Therapist to join our team at Providence Home and Community Care (HCC) in Oregon. As an Occupational Therapist, you will work with patients and clients with a wide range of diagnoses and disabilities in a home setting, providing skilled occupational therapy in accordance with a physician-directed plan of treatment.

Key Responsibilities
  • Manage and provide skilled occupational therapy to patients and clients with various diagnoses and disabilities in a home setting.
  • Develop and implement individualized plans of care to meet the unique needs of each patient.
  • Collaborate with healthcare teams to ensure comprehensive care and coordination.
  • Provide education and training to patients and caregivers on occupational therapy techniques and strategies.
  • Document patient progress and outcomes in accordance with regulatory requirements.
Requirements
  • Bachelor's Degree from an accredited Occupational Therapy Program or Master's Degree from an accredited Occupational Therapy Program or Ph.D. from an accredited Occupational Therapy Program.
  • National Provider BLS - American Heart Association within 30 days of hire.
  • Oregon Occupational Therapist License upon hire.
  • Washington Occupational Therapist License upon hire if practicing within this state.
