- Responds promptly to patient inquiries and requests.

3. Clinical Responsibilities:

- Executes nursing duties with integrity and quality under the supervision of the Director of Nursing.

- Conducts nursing visits, which may involve STI testing, pregnancy assessments, injections, and other clinical tasks as directed.

- Performs point-of-care laboratory tests and manages specimen processing.

- Completes intake interviews to assess client needs.

- Reviews treatment plans and objectives established by healthcare providers.

- Develops nursing care plans in accordance with agency policies and established protocols.

- Coordinates treatment schedules and resources, including transportation.

- Monitors care continuity and evaluates treatment responses.

- Ensures quality care by adhering to established therapeutic standards and making necessary adjustments.

- Administers medications following best practices.

- Conducts triage for patient inquiries via phone or in-person.

- Maintains accurate records of services and program activities.

4. Supervisory Duties:

- Conducts performance evaluations for direct reports.

- Organizes and delegates tasks among clinical team members.

- Monitors team performance to ensure timely and accurate completion of duties.

- Recommends disciplinary actions in collaboration with the Director of Nursing.

- Ensures compliance with clinical policies and procedures.

5. Patient Education:

- Provides education on preventive health topics, including nutrition and immunizations.

- Educates patients on medication adherence and health maintenance.

- Offers guidance on pediatric safety and developmental milestones.

6. Safety and Infection Control:

- Adheres to Universal Precautions at all times.

- Conducts patient care in a manner that prioritizes safety and minimizes errors.

- Reports and documents incidents promptly.

7. Professional Development:

Qualifications:

- Bachelor of Science or Associate degree in Nursing with a valid RN license in Louisiana.

Experience and Skills:

- 3-5 years of experience in case management, preferably in infectious disease or behavioral health.

- Current Basic Life Support (BLS) Certification is mandatory.

Physical Requirements:

- The role involves standing, sitting, and using hands for computer and telephone tasks.

- Light to moderate lifting may be necessary.

Work Environment:
