Job Responsibilities- Deliver High-Quality Patient Care: As a Per Diem RN, you will be responsible for delivering high-quality patient care to individuals in their own homes. This will include assessing patients' needs, developing and implementing individualized care plans, and providing ongoing support and education to patients and their families.
- Collaborate with Healthcare Team: You will work closely with other healthcare professionals, including physicians, therapists, and social workers, to ensure that patients receive comprehensive and coordinated care.
- Communicate Effectively: Strong communication skills are essential for this role, as you will need to communicate effectively with patients, families, and healthcare team members to ensure that care is delivered in a timely and effective manner.
- Document Patient Information: You will be responsible for documenting patient information, including medical history, treatment plans, and progress notes, in a timely and accurate manner.
- Active RN License: You must have an active and current license to practice as a registered nurse in the state of Pennsylvania.
- Experience in Home Health Care: While experience in home health care is preferred, it is not required. We will provide training and support to ensure that you have the skills and knowledge needed to succeed in this role.
- Strong Communication Skills: Excellent communication skills are essential for this role, as you will need to communicate effectively with patients, families, and healthcare team members.
- Ability to Travel: You must be able to travel to patients' homes as assigned.
