* Attend Wellness Health Fairs and other community events to promote Annual Wellness Interviews

* Demonstrate strong communication skills, articulating the objectives and benefits of community-based patient care, and maintaining collaborative working relationships with medical staff, nursing, and other disciplines involved in patient care throughout the continuum
