Overview: The Clinical Nurse Specialist for Advanced Lung Disease plays a pivotal role as the clinical liaison within our ALD Program, focusing on outpatient care. Under the guidance of ALD providers, this specialized nurse will oversee clinical operations in alignment with established protocols and directives.

Key Responsibilities:
  • Manage incoming triage communications, conducting clinical evaluations and addressing symptoms and side effects associated with treatments.
  • Perform comprehensive nursing assessments to collect vital data and clinical insights to support patient care.
  • Facilitate scheduling of urgent visits with healthcare providers as necessary, ensuring accurate documentation in medical records.
  • Deliver empathetic and targeted nursing assistance to patients, their families, and caregivers, while providing educational resources to enhance understanding.
  • Utilize clinical judgment grounded in evidence-based practices, policies, and guidelines for informed decision-making.
Assist in onboarding new patients by compiling necessary documentation and diagnostic results prior to appointments. Coordinate referrals to external services, including hospice, palliative care, and lung transplant options.

Engage in planning and coordination for various pulmonary support initiatives, including groups focused on pulmonary hypertension and pulmonary fibrosis. Participate in meetings with group members to foster community support.

Oversee the scheduling of laboratory tests and medication adjustments for patients on high-risk therapies, adhering to established protocols. Report any irregular findings to ALD providers and ensure appropriate patient follow-up and documentation.

Assist with the enrollment process for specialty pharmacy medications, including prior authorizations, insurance appeals, and financial aid support. Manage prescription refills as required.

Act as a representative for Piedmont Healthcare at national nonprofit organizations and accrediting entities, such as the Pulmonary Fibrosis Foundation and the Pulmonary Hypertension Association. Participate in relevant committees and attend national conferences and meetings.

Qualifications:
Minimum Education Required:
Completion of an accredited registered nursing program or a bachelor's degree in nursing.

Minimum Experience Required:
No prior experience necessary.

Minimum Licensure/Certification Required by Law:
Registered Nurse licensure in the State of Georgia is mandatory.
