Job Summary

We are seeking a highly organized and compassionate individual to fill the role of Senior Program Manager and Coordinator. The ideal candidate will have experience managing programs and services for individuals with disabilities, ensuring compliance with Minnesota's 245D Home and Community-Based Services standards.

The Senior Program Manager and Coordinator will be responsible for overseeing the provision of person-centered services, coordinating staff, and maintaining high-quality care for individuals receiving services.

Key Responsibilities:

  • Ensure compliance with 245D licensing standards, including service planning, staff qualifications, and service delivery.
  • Oversee the day-to-day management of services provided to individuals with disabilities.
  • Develop and implement policies and procedures to maintain quality services in accordance with state and federal regulations.
  • Train, supervise, and evaluate direct support staff to ensure they meet the necessary qualifications and performance standards.
  • Manage incident reporting, tracking corrective actions, and ensuring resolution in a timely manner.
  • Monitor service delivery to ensure services are delivered according to the individual's support plan.
  • Collaborate with case managers, families, and other stakeholders to ensure individual needs are met.

Qualifications:

  • Education: Bachelor's degree in social work, psychology, nursing, or a related field. Equivalent experience may be considered.
  • Experience: Minimum of 2 years of experience in a supervisory or managerial role within a 245D licensed program or similar environment.
  • Licensure/Certifications: Must meet the qualifications for a Designated Manager and Designated Coordinator as outlined in Minnesota Statute 245D.
  • Knowledge: Strong understanding of 245D regulations, person-centered practices, and best practices for serving individuals with disabilities.
  • Skills: Excellent leadership, organizational, and communication skills. Ability to effectively manage multiple tasks and priorities.
  • Other Requirements: Valid driver's license, reliable transportation, and the ability to pass a background check.
