Job Title: Community Paramedic

Clackamas County's Public Health Department is seeking a highly skilled Community Paramedic to join our team. As a Community Paramedic, you will play a critical role in providing and connecting patients to primary care and other services, completing post-hospital follow-up care, and promoting health education.

Job Summary:

The Community Paramedic will work collaboratively with community partners to develop and implement community-based programs that improve health outcomes and reduce healthcare costs. This includes providing medical outreach services, evaluating and treating patients, and coordinating care with healthcare providers.

Responsibilities:
  • Collaborate with community partners to develop and implement community-based programs that improve health outcomes and reduce healthcare costs.
  • Provide medical outreach services, including evaluation and treatment of patients, and coordination of care with healthcare providers.
  • Conduct post-hospital follow-up care, including general physical exams, history and physical exams, and daily or weekly weight checks.
  • Observe, record, and report patient conditions, reactions to drugs, treatment, and significant incidents.
  • Conduct patient education on topics such as diabetes prevention and treatment, hypertension, congestive heart failure, and fall prevention.
  • Perform post-injury follow-up, including fall prevention activities such as assessing for grab bar placements.
  • Coordinate appointments and follow-ups with physicians and hospitals.
  • Connect individuals to community resources.
  • Complete required reports and documentation.
Requirements:
  • Minimum of three (3) years of related experience as a practicing Paramedic.
  • Oregon Paramedic License (required at time of hire).
  • Community Paramedic (CP-C) Certification (preferred).
  • Critical Care Paramedic (CCP-C) Certification (preferred).
  • Oregon Qualified Mental Health Associate (QMHA) Certification (preferred).
  • Oregon Community Healthcare Worker Certification (preferred).
  • Case Management experience working with allied health, county health, and social services.
  • Bilingual English/Spanish proficiency (preferred).
Working Conditions:

Duties are performed outdoors in a variety of conditions, including excessive temperatures, inclement weather, and exposure to various hazardous chemicals. Incumbents work with varied socioeconomic groups, the mentally ill, the developmentally disabled, alcohol and substance abusing clients, and their families.

Positions require physical exertion and agility, including stooping, bending, kneeling, pushing, pulling, and lifting objects and people.

Work Schedule:

This position is included in the County's alternate workweek program, working 40 hours during a standard workweek of Monday through Thursday (Fridays off). Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.

This position is eligible for hybrid on-site/telework subject to the Department's business needs. The exact amount of on-site/telework and schedule will be discussed with the hiring manager during the time of offer. Telework locations cannot be located outside of Oregon or Washington. Washington residents must perform 50% of their time on-site at Clackamas County.
