About the Role

At Professional Management Enterprises, we are seeking a highly skilled and experienced Care Coordinator to join our team. As a Care Coordinator, you will play a critical role in overseeing the care coordination and complex case management functions for our members who live in the community and are not receiving Home and Community Based Services (HCBS) or designated as Nursing Facility Level of Care (NFLOC).

Key Responsibilities
  • Care Coordination: Develop and implement individualized care plans that meet the unique needs of our members, ensuring the most appropriate and least restrictive level of care.
  • Team Leadership: Supervise and mentor a team of care coordinators, providing guidance and support to ensure the delivery of high-quality care services.
  • Communication: Collaborate with healthcare providers, members, and other stakeholders to ensure seamless communication and coordination of care services.
  • Process Improvement: Identify and implement process improvements to enhance the efficiency and effectiveness of our care coordination program.
  • Quality Assurance: Conduct regular quality reviews and audits to ensure compliance with program requirements and regulatory standards.
Requirements
  • Education: Bachelor of Science in Nursing (BSN) or equivalent experience.
  • Licensure: Registered Nurse with an unrestricted license in Indiana.
  • Experience: At least 3 years of experience in a healthcare setting, with a focus on care coordination and case management.
  • Skills: Strong communication and leadership skills, with the ability to navigate complex healthcare systems and prioritize multiple tasks and responsibilities.
Preferred Qualifications
  • Leadership Experience: 3+ years of experience in a leadership role, with a focus on care coordination and case management.
  • Managed Care Experience: Experience working within a managed care environment, with a focus on care coordination and case management.
  • Certifications: Certified Case Manager (CCM) or equivalent certification.
