RN (Per Diem/On Call) - PACE/Elderplace Care Manager

We are seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Per Diem/On Call RN for our Elderplace program. As a key member of our interdisciplinary team, you will provide high-quality care management, care coordination, and skilled care services to our patients with complex medical and psychosocial needs.
Key Responsibilities- Provide care management and care coordination services to patients with complex medical and psychosocial needs.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Provide skilled care services, including wound care, medication management, and vital sign monitoring.
- Travel between multiple locations to provide care services to patients in their homes and communities.
- Current Oregon Registered Nurse License.
- National Provider BLS - American Heart Association certification within 30 days of hire.
- Relevant certifications applicable to the position, as specified by the Nurse Manager.
- Bachelor's Degree in Nursing.
- 1 year of experience in home health nursing.
