We are seeking a compassionate and skilled Registered Nurse to join our interdisciplinary team at Suncrest Home Health & Hospice. As a Registered Nurse, you will play a vital role in providing exceptional care and service to our patients and their families.

About the Role

The Registered Nurse will be responsible for providing care and services for a group of patients, visiting patients in their homes, skilled nursing facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources.

Responsibilities
  • Provide high-quality patient care and services, including psych-social assessments and interventions
  • Collaborate with the interdisciplinary team to develop and implement patient care plans
  • Work closely with patients and their families to understand their needs and preferences
  • Manage patient records and documentation, including electronic medical records
  • Communicate effectively with patients, families, and healthcare professionals
Requirements
  • Current Registered Nurse (RN) licensure in good standing
  • Ability to work independently and as part of a team
  • Strong communication and interpersonal skills
  • Ability to learn and use electronic medical records systems
  • Reliable transportation and a valid driver's license
