Responsibilities- Provide Direct Patient Care: Assess patient needs, develop and implement individualized care plans, and provide ongoing monitoring and evaluation of patient progress.
- Collaborate with Healthcare Team: Work closely with physicians, nurses, and other healthcare professionals to ensure comprehensive and coordinated patient care.
- Conduct Health Education: Educate patients and their families on healthy habits, disease prevention, and management of chronic conditions.
- Participate in Quality Improvement: Contribute to quality improvement initiatives and participate in ongoing education and training to stay current with best practices.
- Maintain Accurate Records: Document patient interactions, treatments, and outcomes in a timely and accurate manner.
- Education: Graduate of an accredited nursing program.
- Experience: Minimum of 2 years of experience as a Nurse Practitioner in a healthcare setting.
- Certifications: Current certification as a Nurse Practitioner in a relevant specialty.
- Skills: Excellent communication and interpersonal skills, ability to work effectively in a fast-paced environment, and strong analytical and problem-solving skills.
