Staff Nurse, LPN

The Staff Nurse, LPN is a vital member of our healthcare team at United Church Homes, Inc. This role is responsible for providing high-quality nursing care to our residents, ensuring their physical, emotional, and social needs are met. As a Licensed Practical Nurse, you will work closely with our interdisciplinary team to coordinate care delivery, develop and implement care plans, and provide education to residents and their families.
Key Responsibilities
- Coordinate care delivery interventions for residents in assigned areas, ensuring adherence to professional practice standards, physician orders, and community policies.
- Assess residents' needs, develop and implement care plans, and evaluate their effectiveness.
- Administer medications, treatments, and other interventions as prescribed, ensuring accurate documentation and adherence to protocols.
- Provide education to residents and their families on health management, disease prevention, and wellness strategies.
- Collaborate with the healthcare team to ensure seamless care transitions and effective communication.
- Participate in quality improvement initiatives, identifying areas for improvement and contributing to the development of solutions.
Requirements
- Successful completion of a state-approved pre-licensure nursing education program.
- Active LPN license conferred by the State Board of Nursing.
- One year of experience in nursing and/or long-term care, preferred.
- Computer skills, including knowledge of basic office equipment and software.
- Certifications and licenses as required by state and federal regulations.
