Position Overview:

Our services encompass treatment for conditions such as post-concussion syndrome, stroke, memory impairment, neurobehavioral challenges, and injuries related to accidents or sports.

Work Schedule: Part-time on weekends, Saturday and Sunday from 8 AM to 9 PM.

Key Responsibilities:

We are in search of part-time Licensed Practical Nurses (LPNs) to join our residential care team. The LPN will operate under the supervision of a registered nurse, providing essential nursing and facilitation services to designated clients. As a member of the Rehabilitation Team, the LPN will engage in and execute evidence-based treatment methodologies that enhance self-care and promote overall physical, psychosocial, and spiritual well-being.

Core Duties & Responsibilities:
  • Identify clients requiring medical nursing services promptly and deliver necessary interventions.
  • Utilize specialized knowledge and clinical expertise to provide comprehensive care, employing the nursing process effectively.
  • Educate and train families, funders, and significant others, utilizing appropriate resources to create and implement personalized teaching and discharge plans.
  • Collaborate with interdisciplinary team members to coordinate nursing care activities that align with treatment objectives.
  • Contribute to the nursing assessment of clients and their families, addressing educational and discharge requirements.
  • Implement the interdisciplinary care plan within the LPN scope of practice, providing nursing care and education in collaboration with Brain Injury specialists as necessary.
  • Assist in evaluating nursing care and modifying plans to achieve measurable outcomes.
  • Communicate essential medical and nursing information to relevant team members, physicians, and healthcare providers.
  • Stay informed about client programs and ReMed updates through various communication channels.
  • Ensure consistent application of strategies by training team members and gathering feedback.
  • Adhere to professional treatment standards, licensure requirements, and state regulations.
  • Support clients during challenging situations, including refusals of activities or medications.
  • Provide direct clinical care as required.
  • Participate in staff training and orientation regarding health-related topics.
  • Balance direct client care, staff communication, and documentation responsibilities effectively.
  • Complete all necessary medical documentation, including progress notes and medication records.
  • Document each nursing session accurately and timely.
  • Conduct nursing assessments and evaluations according to established timelines.
  • Engage in collaborative problem-solving with the team to support clinical decision-making.
  • Maintain current professional certifications and engage in continuing education to stay updated on developments in the field.

Qualifications:

  • Current PA licensure as a Licensed Practical Nurse is mandatory.
  • Experience in the rehabilitation of chronically or catastrophically ill or injured individuals is preferred.
  • Strong accountability and skills in analysis, problem-solving, decision-making, and time management are essential.
  • Effective written and verbal communication skills with families, clients, staff, and physicians are required.
  • A valid driver's license and responsible driving behavior must be maintained.
