Key Responsibilities- Develop and implement personalized care plans in accordance with client needs and goals.
- Conduct home visits to clients in designated geographic territories, providing skilled nursing care and support.
- Administer medication, perform wound care, and provide treatments and procedures as prescribed by physicians.
- Monitor client conditions, reporting changes to Clinical or Client Services Manager.
- Follow up with, execute, and properly document doctors' orders.
- Perform client assessments as necessary and provide case management and coordination.
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided.
- A current license as a Registered Nurse in Vermont.
- Graduation from an accredited and approved nursing program.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills and solid computer skills, with prior experience with electronic medical records (EMR) preferred.
- Ability to travel to cases as assigned.
