Job Summary

BAYADA Home Health Care is seeking a compassionate and dedicated Registered Nurse (RN) to join our team in providing high-quality home health care services to our adult and geriatric clients.

Key Responsibilities
  • Develop and implement personalized care plans in accordance with client needs and goals.
  • Conduct home visits to clients in designated geographic territories, providing skilled nursing care and support.
  • Administer medication, perform wound care, and provide treatments and procedures as prescribed by physicians.
  • Monitor client conditions, reporting changes to Clinical or Client Services Manager.
  • Follow up with, execute, and properly document doctors' orders.
  • Perform client assessments as necessary and provide case management and coordination.
  • Accurately document observations, interventions, and evaluations pertaining to client care management and services provided.
Requirements
  • A current license as a Registered Nurse in Vermont.
  • Graduation from an accredited and approved nursing program.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal skills and solid computer skills, with prior experience with electronic medical records (EMR) preferred.
  • Ability to travel to cases as assigned.
