Job Title: Registered Nurse Home Health

VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our Home Health team in Verona, New Jersey.

About the Role:

As a Registered Nurse Home Health, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence, dignity, and comfort. Your expertise will be essential in assessing patient needs, developing personalized care plans, and collaborating with healthcare teams to ensure seamless care transitions.

Responsibilities:
  • Conduct thorough assessments of patients' physical, emotional, and social needs
  • Develop and implement individualized care plans to address patient goals and outcomes
  • Provide skilled nursing care, including wound care, medication management, and education
  • Collaborate with healthcare teams, including physicians, therapists, and social workers, to ensure comprehensive care
  • Document patient progress, outcomes, and care plans in a timely and accurate manner
  • Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements:
  • Current RN licensure in New Jersey
  • 1+ year of experience in home health or a related field
  • Excellent communication and interpersonal skills
  • Strong organizational and time management skills
  • Ability to work independently and as part of a team
