Registered Nurse

VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our home health team. As a Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to develop and implement individualized care plans.
Responsibilities:- Assess and evaluate patient needs, developing and implementing effective care plans to promote optimal health outcomes.
- Provide skilled nursing care, including wound care, medication management, and education to patients and their families.
- Collaborate with our interdisciplinary team to coordinate care and services, ensuring seamless transitions and continuity of care.
- Document patient progress, identifying areas for improvement and implementing changes as needed.
- Participate in ongoing education and training to stay current on best practices and industry developments.
- Current RN licensure in the state of New Jersey.
- 1+ year of experience in home health or a related field.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Strong problem-solving and critical thinking skills, with the ability to prioritize tasks and manage multiple responsibilities.
- Ability to work independently and as part of a team, with a focus on providing exceptional patient care and service.
