At The Homestead, we are seeking an experienced Senior Nursing Professional to join our dedicated team. This challenging role will provide a unique opportunity for the successful candidate to demonstrate their skills and passion for delivering high-quality patient care.

Job Overview

This position involves working as part of a multidisciplinary team to deliver exceptional patient-centered care to individuals with complex health needs. As a key member of our team, you will be responsible for assessing, planning, implementing, and evaluating the nursing care required by patients in accordance with established policies and procedures.

In this role, you will have the opportunity to work closely with patients, families, and healthcare teams to develop individualized care plans that address the unique needs and goals of each patient.

You will also be responsible for providing education and support to patients and their families, as well as participating in quality improvement initiatives to enhance the overall quality of patient care.

Key Responsibilities
  • Assess patients' physical and emotional needs, identifying areas requiring intervention and developing appropriate care plans.
  • Implement and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal patient outcomes.
  • Collaborate with interdisciplinary teams to coordinate patient care, ensuring seamless transitions between departments and services.
  • Provide education and support to patients and their families, promoting self-advocacy and empowerment.
  • Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices and protocols.
Requirements
  • Diploma or degree in Nursing (RNs or LPNs/LVNs) with current licensure to practice in the relevant state.
  • Minimum one year of experience in long-term care settings, preferably with a focus on gerontological or palliative care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and stakeholders.
  • Ability to work autonomously and as part of a team, prioritizing tasks and managing time efficiently in a fast-paced environment.
  • Strong problem-solving and critical thinking skills, with a commitment to continuous learning and professional development.
