About the Role:

We're seeking a skilled Private Duty Nurse LVN to join our team. As a Private Duty Nurse, you'll work one-on-one with patients in their homes, providing personalized care and support. Your expertise will help our patients transition from hospital to home and thrive in their own environments.

Responsibilities:

  • Implement physicians' prescribed plans of care
  • Conduct patient assessments and coordinate care
  • Administer prescribed medication, treatments, and therapies
  • Document care provided in our system of record
  • Teach and train family members on health promotion and disease management

Qualifications:

  • Active, unencumbered license (RN/LPN/LVN) in the state of practice
  • Current CPR certification from American Red Cross or American Heart Association
  • TB skin test (current within last 12 months)
  • At least six months of prior hands-on nursing experience
