Key Responsibilities
  • Conduct comprehensive in-home assessments to identify patient needs and develop individualized treatment plans.
  • Collaborate with healthcare providers to coordinate care and ensure seamless transitions between settings.
  • Provide education and support to patients, families, and caregivers on health promotion, disease prevention, and management strategies.
Requirements and Qualifications
  • A minimum of 2 years of experience as a Physician's Assistant, preferably in a home wellness setting.
  • Unrestricted prescribing and dispensing privileges with DEA licensure required.
  • Ability to drive and travel to patient locations as needed.
