Gentiva is seeking a highly skilled RN Clinical Liaison to join our team of compassionate healthcare professionals. As a RN Clinical Liaison, you will play a critical role in educating healthcare professionals about hospice services and providing end-of-life solutions that prioritize clinical excellence, compassion, and dignity.

Key Responsibilities
  • Ensure effective and timely intake and referral processes to meet the needs of patients, families, and referral sources.
  • Complete admissions processes for referred patients in emergency cases, as staffing indicates, and for inpatient hospice admissions.
  • Provide accurate and timely information regarding hospice services to inquiring individuals.
  • Respond to calls from referral sources, initiate paperwork, and begin the intake/referral process.
  • Serve as a patient advocate in evaluating and developing plans of care and obtaining informed consent for hospice.
  • Call on multiple account types, including SNFs, ALFs, hospitals, and physicians, to offer in-services and education on the benefits of hospice.
Requirements
  • A RN license in the state(s) of employment.
  • A minimum of 2 years of healthcare experience in one of the following settings: hospice, home health, clinical liaison, RN liaison, clinical coordination, discharge planner, hospice case manager, or RN educator.
  • Known and respected within your community.
