Job Summary

We are seeking a skilled Nurse Practitioner - Float to join our team at HealthCare Support. As a key member of our healthcare team, you will provide high-quality patient care and support to patients with complex medical needs.

Key Responsibilities
  • Conduct comprehensive assessments and develop effective care plans
  • Manage acute and chronic medical conditions, including behavioral health
  • Ensure accurate and complete documentation, including ICD-10 coding
  • Communicate and collaborate with interdisciplinary care teams
  • Provide advanced illness and end-of-life care planning conversations
  • Counsel and educate patients and caregivers on health management
Requirements
  • Clear and active PA NP license
  • 1-2 years of experience in a healthcare setting
  • Preferred qualifications: experience with complex patient populations, geriatrics, and chronic illness management
