Job Summary

We are seeking a skilled Nurse Practitioner - Float to join our team at HealthCare Support. As a key member of our healthcare team, you will provide high-quality patient care and support to patients with complex medical needs.

Key Responsibilities
  • Conduct comprehensive assessments and develop effective care plans for patients with acute and chronic medical conditions.
  • Collaborate with the interdisciplinary care team to ensure seamless patient care and communication.
  • Provide advanced illness and end-of-life discussions, as well as counseling and education to patients and caregivers.
  • Manage patient documentation, including ICD-10 conditions, and maintain accurate and complete records.
Requirements
  • Clear and active PA NP license.
  • 1-2 years of experience in a healthcare setting.
  • Preferred qualifications include experience in geriatrics and chronic illness, as well as advanced illness and end-of-life discussions.
