LPN Visit Nurse Job Summary

Lifesprk is a leading provider of senior health services, and we're seeking a skilled LPN Visit Nurse to join our team. As a LPN Visit Nurse, you will provide direct client care under the supervision of a registered nurse, implementing plans of care and providing accurate and timely documentation.

Responsibilities:

  • Follow the plan of care and provide treatments as defined in the state Nurse Practice Act.
  • Implement plans of care initiated by the registered nurse and provide accurate and timely documentation.
  • Assess and provide client and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
  • Assist with monitoring visits as directed by the RN Case Manager to evaluate ongoing client needs and quality of care.
  • Complete nursing assessments during personal cares; document results; escalate to RN Case Manager as necessary for follow-up.
  • Attend MD appointments as requested and assist with skilled services as directed by care plan.
  • Timely completion of required clinical documents, forms, reports, orders.
  • Assist clients with implementation of Activities of Daily Living and Delegated Tasks according to clients' needs, wishes, and preferences outlined on the Care Assignment.
  • Lead by example in establishing rapport with clients, clinicians, partners, and the community.
  • Ensure positive/professional communication within the team and customer service resolution; escalate concerns.
  • Communicate with the physician regarding the client's needs and reports any changes in the client's condition; obtains/receives physician's orders as required.
  • Communicate with community health related persons to coordinate the care plan.
  • Completes orientation and annual training requirements as assigned and participates in Lifespark's Event Management process including initiating reports, escalation of events and investigations.
  • Promotes safety in the workplace through real time coaching and participation in safety awareness training.
  • Manages client supplies assuring appropriate DME provider is utilized.

Qualifications:

  • Prefer minimum of 1 year of nursing experience.
  • Minimum of 1 year of caregiving experience (assisted living, home care, hospice).
  • Minimum of 1 year experience working with people in the second half of life.
  • Experience in customer service/direct member contact field preferred.
  • Current Minnesota Nursing License.
  • Aging process and related care issues.
  • Assessment and person-centered care planning.
  • Caregiver and client education.
  • Strong written and oral communication skills.
