Position Details
This is a full-time salaried role with a Monday to Friday schedule, allowing for a work-life balance without the demands of weekends, evenings, on-call duties, or holidays. Daily local travel to patients' residences is a requirement, with all mileage reimbursed. Necessary equipment, medical supplies, and training will be provided.
Key Responsibilities
- Conduct thorough assessments of health status for patients requiring wound care, utilizing advanced practice knowledge and skills.
- Develop and implement personalized care plans, ensuring coordination of care between visits.
- Perform comprehensive patient evaluations, focusing on factors that may contribute to wound formation.
- Administer advanced treatments, including surgical debridement, UltraMIST therapy, and skin substitutes.
- Manage medication orders and provide clear usage instructions.
- Educate nursing staff and maintain effective communication with primary care providers, families, and clinical teams as necessary.
- Contribute to the development of efficient practice management policies that enhance patient access to care.
- Demonstrate the ability to operate at the highest level within the scope of practice.
- Engage in patient education and proactive disease management.
- Exhibit a commitment to exemplary patient care and adapt approaches to meet the developmental needs of the population served.
Qualifications
- Graduation from an accredited nurse practitioner or physician assistant program.
- A clinical background in adult, family, geriatric, podiatry, or vascular specialties.
- A minimum of one year of experience as an advanced practice provider.
- Board Certification/Preparation through AANP, ANCC, or NCCPA.
- Experience in wound care is advantageous.
- National certification as an ANP, FNP, or GNP is required or must be obtained.
- Possession of a current, unrestricted license in the state of assignment or the ability to obtain one.
- Capability to obtain and/or maintain State DEA licensure and Prescriptive Authority where applicable.
- Familiarity with Microsoft Office and CRM systems is preferred.
