Advanced Practice Provider

Job Summary
Hartford Healthcare Medical Group is seeking a licensed Advanced Practice Provider to join our Gynecologic Oncology team. This is an excellent opportunity to work in a collaborative team environment where all providers are connected through Epic, our fully-integrated EMR system. The ideal candidate will provide holistic, quality and cost-effective advanced practice care for patients, families, and others in a target population, and participate in the medical management of patients to achieve or maintain optimal health status, manage disease and reduce risk of illness.
Key Responsibilities
- Evaluate and assess patient illness/injuries
- Perform physical examinations and special examinations; take medical histories
- Develop plans of care
- Intervene in crisis situations or patient deterioration; provide routine and illness care
- Order medications; order diagnostic tests and evaluate results
- Assume responsibility for patient follow-up, coordination, and referrals to other health care providers as necessary
- Maintain patient medical records
- May develop policies/procedures; participate in or design changes in the system of care delivery
- May participate in clinical research in conjunction with Memorial Sloan Kettering Cancer Alliance
- Provide patient counseling/education
- Participate in medical student and health professional teaching; act as a clinical consultant to peers and other health care professionals
Requirements
· Completion of Approved Nurse Practitioner or Physician Assistant Program
· Current APRN license or Physician Assistant license required
· Certification according to CT state law
· Master's Degree or PhD in Nursing or PA program strongly preferred
· Any of the following experience strongly preferred
· 1-3 years of experience with oncology patients
· 1-3 years' experience with patients undergoing chemotherapy
· Experience with palliative/hospice care
