Job Title: Direct Support Professional

Achievement Health Group, LLC is seeking a compassionate and dedicated Direct Support Professional to join our team. As a Direct Support Professional, you will play a vital role in providing quality care and support to individuals with disabilities, helping them to achieve their personal outcomes and live fulfilling lives.

Job Summary:

The Direct Support Professional will work under the supervision of the House Manager or Program Coordinator, providing direct care and support to individuals in their homes. This includes assisting with daily living activities, such as bathing, dressing, and grooming, as well as providing transportation and meal preparation. The ideal candidate will be able to work independently, think critically, and communicate effectively with individuals, families, and colleagues.

Responsibilities:
  • Provide quality care and support to individuals with disabilities, including assistance with daily living activities, transportation, and meal preparation.
  • Develop and implement individualized care plans, in collaboration with the individual, their family, and other healthcare professionals.
  • Monitor and report any changes in the individual's health or well-being, and provide emergency care as needed.
  • Assist individuals with medication management, including administration and monitoring.
  • Provide emotional support and companionship to individuals, promoting their independence and self-esteem.
  • Communicate effectively with individuals, families, and colleagues, including reporting any concerns or issues.
Requirements:
  • High School Diploma or equivalent required.
  • CPR and First Aid certification required.
  • CNA license preferred, but not required.
  • Ability to lift, bend, and stand for long periods, as well as work in a fast-paced environment.
  • Excellent communication and interpersonal skills, with the ability to work effectively with individuals, families, and colleagues.
