Job Summary

We are seeking a skilled and compassionate Nurse Care Specialist to join our team at Delta HomeCare. As a key member of our healthcare team, you will provide high-quality nursing care to patients in their homes, community, and residential settings.

Responsibilities
  • Provide skilled visits and nursing care to patients, including diagnostic procedures and lab tests as permitted.
  • Assist patients in organizing their physical activities and provide personalized care services.
  • Conduct supervisory visits to monitor patient care and provide guidance to caregivers.
  • Assess patients' health needs, create care plans, and facilitate insurance assessments.
  • Collaborate with office staff, caregivers, and other healthcare professionals to ensure seamless care delivery.
Requirements
  • Graduate of an accredited school of professional nursing.
  • Current licensure as a Registered Nurse or LPN through the Tennessee State Board of Nursing.
  • At least two years of general nursing experience, with current home care, medical, surgical, or critical experience.
  • Excellent communication and interpersonal skills.
  • Ability to work independently, multitask, and meet deadlines.
