Job Title: MDS Coordinator RN/LPN

Laurel Park Rehabilitation and Healthcare Center is seeking a skilled MDS Coordinator RN/LPN to join our team.

Job Summary:

We are looking for a detail-oriented and organized MDS Coordinator RN/LPN to assist with the development of comprehensive care plans for our residents. The successful candidate will have a strong understanding of federal and state regulations, as well as professional standards.

Key Responsibilities:
  • Assist with the development of care plans that accurately address the needs of our residents.
  • Coordinate and manage the written plan of care for each resident.
  • Monitor resident status changes and ensure timely nursing or clinical involvement.
  • Perform regular audits of documentation to ensure accuracy.
  • Assist with the discharge process and perform administrative requirements.
Requirements:
  • Current State License as a Nurse
  • C.P.R. Certified
Preferred Qualifications:
  • One year experience as a Nurse in a long-term care setting
  • Knowledge and training in all aspects of MDS process
