We are seeking a skilled MDS Coordinator RN/LPN to join our team in a part-time capacity. As a key member of our healthcare team, you will play a vital role in ensuring the highest quality of care for our residents.
Key Responsibilities:- Assist with the development of comprehensive care plans for residents in coordination with the MDS, accurately addressing their needs.
- Coordinate, manage, and monitor the written plan of care for each resident, identifying their needs and goals.
- Assist nursing management with the coordination, management, and review of nurse's notes to determine if the care plan is being followed.
- Monitor resident status changes to ensure timely nursing or clinical team involvement.
- Ensure MDS and support documentation are accurate representations of the resident and meet regulatory and auditor requirements.
- Current State License as a Nurse
- C.P.R. Certified
- One year experience as a Nurse in a long-term care setting
