Compensation and Work Schedule:
  • Hourly rate: $45/hr - $55/hr
  • Part-time position with potential for full-time
  • Workdays: Monday to Friday
  • Day shifts available
  • Weekend shifts as needed
Key Responsibilities:
  • Deliver physician-recommended physical therapy services.
  • Assist patients in improving or minimizing physical disabilities.
  • Help individuals achieve their highest possible level of functionality.
  • Collaborate with home care staff in developing patient care plans.
  • Oversee and guide other personnel as necessary.
  • Conduct initial assessments, create care plans, and perform monthly reassessments.
Qualifications:
  • A strong desire to assist others in achieving their best quality of life.
  • Degree in physical therapy (Bachelor's, Master's, or Doctorate) from an accredited program.
  • Current state licensure for practice.
  • Preferred: Two years of experience in a home health environment.
  • Familiarity with Medicare OASIS documentation is advantageous.
