Home Healthcare Provider for Patient Visits in Houston

About the Role
We are seeking a skilled Home Healthcare Provider to join our team at Advanced Practice House Calls LLC. This full-time role involves visiting patients in their homes for medical evaluation and management.
Key Responsibilities
- Diagnose and manage acute, chronic, and episodic conditions for patients in their homes
- Prescribe medications and treatments as necessary
- Perform thorough physical examinations to assess patient health
- Develop and implement patient management plans to ensure optimal care
- Facilitate referrals and coordinate care with home health agencies when needed
Requirements
- Certified Nurse Practitioner or Physician Assistant license (required)
- At least 1 year of experience as an advanced practice provider (preferred)
- Spanish-speaking candidates are highly desirable
- Ability to manage daily driving within the specified schedule
Compensation and Benefits
- Estimated salary: $101,000 - $108,000 per year (inclusive of no transport allowance)
- Bonuses: Performance-based bonuses available
- Paid Time Off:
- 6 PTO/Sick days annually
- 9 paid holidays per year
- 1 week paid vacation annually
- 2 paid bereavement days for immediate family
- Insurance: Free malpractice insurance
