About the Role

This role involves providing comprehensive nursing services to support the physical and emotional well-being of our residents. As a Nurse Care Coordinator, you will be responsible for coordinating medication administration, direct individual healthcare programs, and facilitating education for direct care staff in relevant healthcare areas.

Key Responsibilities
  • Coordinate and facilitate medication administration and direct individual healthcare programs for residents
  • Provide education and training to direct care staff on healthcare-related topics
  • Assess resident health needs through admission and annual nurse and health assessments
  • Monitor laboratory tests and examinations, and document medical treatment and health status in resident records
  • Collaborate with interdisciplinary treatment teams to develop healthcare goals and ensure inclusion in Individual Plans of Service (IPOS)
Requirements

To be successful in this role, you will need:

  • A valid Registered Nurse license in the State of Michigan
  • Excellent communication and interpersonal skills
  • A positive attitude and strong work ethic
  • Experience in providing medical treatment services within specialized rehabilitation settings, preferably with mental health experience
