Job Summary

We are looking for a compassionate and experienced Registered Nurse to provide care and services for a group of patients in their private homes, Skilled Nursing Facilities, and assisted living facilities. As a contributing member of our interdisciplinary team, you will be responsible for providing expertise regarding psych-social assessments, interventions, and knowledge of community sources.

Key Responsibilities

  • Provide high-quality patient care and services, including assessments, interventions, and education.
  • Collaborate with our interdisciplinary team to develop and implement patient care plans.
  • Conduct regular visits to patients in their homes, Skilled Nursing Facilities, and assisted living facilities.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Manage and prioritize multiple tasks and responsibilities, including documentation, meetings, and phone calls.
  • Stay up-to-date with the latest hospice rules and regulations, as well as electronic medical records systems.
  • Travel across the service area, using reliable transportation, to visit patients and attend meetings.

Requirements

  • Current Registered Nurse (RN) licensure in the state of Georgia.
  • Previous hospice experience required.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Time management and organizational skills.
