Job Summary

We are seeking a skilled Physical Therapist to join our team at HexaQuEST Health, Inc. as a key member of our Home Health department. The ideal candidate will have a strong background in rehabilitation therapy and a passion for delivering high-quality patient care.

Key Responsibilities
  • Evaluate and treat patients with medically prescribed physical therapy programs
  • Develop and implement individualized treatment plans to achieve patient goals
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care
  • Document patient progress and outcomes in a timely and accurate manner
  • Supervise Physical Therapy Assistants as needed
Requirements
  • Current licensure as a Physical Therapist in the state of California
  • One year of experience in Home Health or a related setting
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
Work Environment

This position will be based in the San Francisco Bay Area, with a focus on serving patients in Sunnyvale, Mountain View, Los Altos, Palo Alto, Stanford, Menlo Park, East Palo Alto, Atherton, and surrounding cities.
