Family Nurse Practitioner

Position Overview:
HexaQuEST Health, Inc. is a well-established, mission-oriented organization dedicated to delivering exceptional healthcare services to a diverse community. Our commitment to patient care is reflected in our team of over 700 professionals working across multiple locations.
We are currently in search of a bi-lingual (Spanish/English) Nurse Practitioner who will play a crucial role in our Patient Centered Healthcare Team. This position involves providing high-quality medical care, conducting assessments, and assisting patients in managing their health conditions.
Key Responsibilities:
- Conduct thorough assessments of patients, including their medical history, current medications, and psychosocial factors.
- Review abnormal lab results and determine appropriate treatment plans in collaboration with the primary physician.
- Identify educational needs for patients and their families, providing tailored teaching and referrals as necessary.
- Prioritize patient concerns based on assessments and develop comprehensive care plans that align with patient goals.
- Collaborate with a multidisciplinary team to ensure holistic care for clients.
- Engage family members in the care process when appropriate, ensuring they are informed and involved.
- Deliver care in accordance with established protocols, continuously evaluating patient responses to treatment.
- Facilitate educational opportunities for patients and their families regarding health and wellness.
- Maintain accurate documentation of patient interactions, educational materials provided, and follow-up needs.
- Uphold professional standards and demonstrate a commitment to nursing excellence through adherence to best practices.
- Participate in the orientation and training of new staff, sharing knowledge of policies and procedures.
- Act as a resource for the healthcare team, providing informational sessions as needed.
- Develop and manage educational resources for both patients and staff.
- Flexibility to travel between various healthcare sites may be required.
Qualifications:
- Minimum of 2 years of recent experience as a Nurse Practitioner.
- Current licensure in Massachusetts.
- Ability to work effectively within a multidisciplinary team.
- Eagerness to learn from others and share knowledge.
- Bi-lingual (Spanish) proficiency is essential.
