Job Summary

We are seeking a skilled and compassionate Home Health Nurse Specialist to join our team. This is a full-time opportunity to provide high-quality nursing care to patients in their own homes, with a focus on promoting independence, improving health outcomes, and enhancing the overall quality of life.

Responsibilities
  • Provide direct patient care, including assessments, develop and implement individualized care plans, and coordinate with other healthcare professionals to ensure seamless care transitions.
  • Conduct thorough assessments of patients' physical, emotional, and social needs, and develop a comprehensive care plan that addresses these needs.
  • Perform skills such as wound care, medication management, and IV therapy, as needed.
  • Collaborate with patients, families, and caregivers to educate them on disease management, medication adherence, and other aspects of care.
  • Document all patient interactions, including assessments, care plans, and progress notes, in a timely and accurate manner.
Requirements
  • License and Certification: Current RN license in the State of Washington, and current BLS certification.
  • Education: Bachelor's degree in Nursing (BSN) required within 5 years of employment.
  • Experience: At least 1 year of experience in home health or a related field.
